What whitening costs and how long it takes

Price tracks with how much supervision and customization the system involves, and with how dramatic a change you want.

Over-the-counter strips cost far less and do lighten surface stains. What they cannot do is hold gel against the tooth in a sealed, custom-fitted environment, which is the difference between removing a film and actually lifting color out of the enamel.

In-office, take-home, or KöR

All three use peroxide. They differ in concentration, contact time, and how well the gel is kept where it belongs.

In-office whitening

We isolate and protect the gums, apply a professional-strength gel, and let it work under supervision. You leave the same day with a visible change, which is why this is the popular choice before a wedding, a reunion, or a photo-heavy trip.

Custom take-home trays

We take impressions and fabricate trays that fit your arch precisely. You wear them with a lower-concentration gel on your own schedule. The pace is slower, the control is excellent, and you keep the trays for future touch-ups, which makes this the better long-term value for most people.

KöR deep bleaching

KöR whitening is the system we reach for when staining runs deeper than a standard treatment will address, including tetracycline-related discoloration and long-set intrinsic color. It combines a conditioning phase, custom trays worn at home, and in-office sessions, and it includes a desensitizing protocol built into the sequence.

What whitening can and cannot fix

This is the part patients most often learn too late, and it is the reason we examine before recommending anything.

Whitening works well on yellowing from age, coffee, tea, red wine, tobacco, and the surface staining that builds up on natural enamel over years.

Whitening does not change crowns, veneers, bonding, or tooth-colored fillings. Those materials hold whatever shade they were made in. If you have a crown on a front tooth, whitening the teeth around it will make the mismatch more obvious, so the sequence matters: whiten first, then match or replace the restoration to the new shade.

Whitening will not lighten exposed root surfaces. Where gums have receded, the exposed root is dentin rather than enamel, and it does not respond the way the crown of the tooth does.

Whitening is not the answer for white spot lesions, fluorosis mottling, or dark single teeth caused by an old trauma or a dead nerve. Those have their own treatments, from microabrasion to internal bleaching to veneers, and whitening can sometimes make spotting more noticeable before it becomes less so.

Whitening is safe for most healthy adults. A few things need to be handled first:

• Active decay or a failing filling. Peroxide reaching the inside of a tooth through a cavity is a painful way to find out it was there.
• Gum disease or significant recession. Inflamed tissue and exposed roots both make the process uncomfortable and the result uneven.
• Existing sensitivity. Worth diagnosing before whitening rather than during. We walk through this in detail in our guide to whitening with sensitive teeth.
• Restorations on the teeth that show. Not a disqualifier, just a sequencing decision.
• Pregnancy or nursing. Usually a reason to postpone elective whitening.
• Unrealistic shade goals. Everyone has a natural ceiling. Part of the consult is showing you what yours looks like.

What to expect at your appointment

Sensitivity, and how we manage it

Temporary sensitivity is the most common side effect of whitening, and it is not a sign something went wrong. Peroxide moves through microscopic channels in the enamel and briefly irritates the nerve, which typically settles within a day or two.

What we do about it: pre-treat with a potassium nitrate toothpaste in the two weeks beforehand when your history calls for it, use fluoride or desensitizing gel after treatment, adjust concentration and wear time rather than pushing through discomfort, and space sessions out when needed. Patients who have struggled with strips often do well with a lower-concentration tray system precisely because we can slow it down.

What causes the staining in the first place

Knowing which kind of discoloration you have predicts your result better than any product claim.

Extrinsic staining

Pigment that sits on the outside of the enamel, deposited by coffee, black tea, red wine, dark sodas, tomato-based sauces, curry, and tobacco. This is the category that responds best and fastest, and part of it lifts off during a cleaning before whitening even starts.

Intrinsic staining

Color that lives inside the tooth structure. It comes from aging, where enamel thins and the naturally yellower dentin beneath shows through more, from certain medications taken during tooth development, and from trauma to a specific tooth. Intrinsic staining responds to whitening, but it takes longer and deeper systems to move, which is the gap KöR is designed to close.

Things that are not stains at all

White spot lesions, fluorosis mottling, and enamel defects are differences in the enamel itself rather than pigment sitting on or in it. Whitening lightens the surrounding tooth, which can make a white spot stand out more before it blends in. We flag this at the consult rather than after.

Making the results last

Whitening is not permanent, and how long it holds depends mostly on what passes over your teeth daily.

• Rinse with water after staining drinks. Coffee, black tea, red wine, and dark sodas are the usual culprits in that order.
• Use a straw for iced coffee and tea, which keeps a surprising amount of pigment off the front teeth.
• Be careful in the first 48 hours. Enamel is temporarily more porous right after treatment, so this is the worst window for deeply pigmented food and drink.
• Keep your trays. A single night of touch-up gel a couple of times a year usually maintains the result indefinitely.
• Stay on your cleaning schedule. Professional cleanings remove the surface film that dulls a whitened smile.
